I assumed that (in this comic) Batman had acne on half of his face like Cueball, but on the upper half. That's why he's wearing a mask covering half of his face. He punches Cueball because he's offended that Cueball plans to give other people half-faced acne, too. Or Batman is a friend of Cueball and he particpated in such a trial which left him with half an acne face over which he has to wear a mask.
