(Though it was previously changed away from this text, this is how it appears. And now looks even more correct. It's not a monospace code/markup-example, but an example of the appearance it will take in the (default) page layout. Other problems left in.)

Step one: Type <code><nowiki>{{YourTemplateName}}</nowiki></code> (but substituting your own name) on whatever page you want your template on.

Step Two: You should see a red line that looks something like <span style="color: #BA0000"><nowiki>Template:YourTemplateName</nowiki></span> that is (unlike here) a link. Click it.

Step Three: Add the content of your template by typing it in wikitext. If you want to use a specific image, make sure to upload it first before linking to it in the template.

Step Four: Add content to the page that should not be included in the template. This can be done with the noinclude and /noinclude commands.

Step Five: Now that you've added the content of your template, it's time to use it! Use it in a page by typing <code><nowiki>{{YourTemplateName}}</nowiki></code> on whatever page you want to use it on. Make sure to not use it on comic pages or comic discussion pages.
